Targeted Case Management (TCM)

Criteria for Admission

 

Milwaukee County resident 18 years and older

Client has a Substance Abuse and/or Mental Health Diagnosis

Clients income is at or below 200% of the Federal Poverty Level

Targeted Case Management (TCM) / Crisis Case Management (CCM)

 

Targeted Case Management (TCM) is a low intensity co-occurring case management model.  This model can be long or short-term to assist the consumer in their recovery path.  TCM includes a primary case manager model with peer specialist services.  Crisis Care Management (CCM) is an intensive case management model. CCM utilizes a team approach to stabilize the crisis occurring in the individuals life.  The team may consist of a supervisor, clinician, case manager, peer specialist, and crisis stabilizer.

Consumers will have the opportunity to receive a varied array of services based on their co-occurring needs.  Consumers can have routine check-in/low intensity case management services with Targeted Case Management.  Consumers in crisis have the option to receive stabilization/high intensity services with Crisis Care Management.  Both programs provide person centered care that assist the consumer on their recovery path.

 

Supportive Services Include - Targeted Case Management

Consumers are provided light case management with face-to-face visits at a minimum of twice a month up to once-a-week.  Basic services are:

  • Refer to community resources
  • Apply for appropriate resources
  • Assessments
  • Case Plans
  • Case Notes

Crisis Care Management

Consumers receive multiple face-to-face visits during the week, all provided by a multidisciplinary team.  Basic services are:

  • Prevent or reduce a crisis
  • Medication Management
  • Assessments
  • Crisis Plan & Response Plan
  • Crisis Supervision
  • Crisis Notes

**The CCM is based on DHS 34 Regulations**

Milwaukee County has 7 outside contracted TCM/CCM agencies. 

If the agency does not provide a needed service, they will connect you with the needed outside resources.
