How We Help
Milwaukee County Mobile Integrated Health (MIH) is a community focused program designed to meet people where they are, both physically and in their healthcare journey. Our team of clinicians work beyond traditional emergency response to provide proactive, personalized care aimed at improving overall health and preventing significant health events, emergency room visits, and hospitalizations. Our goal is simple: to provide compassionate, accessible care that empowers the individual to be safe, healthy, and supported wherever they may be. We can assist in a wide variety of health and social concerns including:
- Chronic conditions management including diabetes, heart failure, and COPD.
- Medication management and reconciliation.
- Connection to primary care, social services, and community resources.
- Substance use and overdose prevention.
- Mental health resource navigation.

How do I know if I'm eligible to receive help?
Any Milwaukee County resident is eligible to receive services from the Mobile Integrated Health (MIH) team.
What if I have an immediate or emergent need?
Always dial 911 for emergencies. Mobile Integrated Health (MIH) does not replace traditional emergency services.

Do I have to pay for any of the services I receive?
Patients are provided services by the Mobile Integrated Health (MIH) team at no cost.
What if my community is served by another Mobile Integrated Health (MIH) team?
Our team works to bridge patients to the services they need regardless of community or municipality. If a referral is received for a patient where a team already exists, they will be connected to the local Mobile Integrated Health (MIH) team.
What kind of training do the members of your team have?
Our Mobile Integrated Health (MIH) team consist of multiple disciplines and a wide variety of training, certification, and licensure. Our team also partners with subject matter experts and community organizations to ensure we are providing culturally competent care to the communities we serve.
