Complete a Cremation View Request Form

Funeral homes located north of I-94 in Milwaukee County will be visited for viewings Monday, Wednesday and Friday, if the request is received before 8 a.m. on that day. Funeral homes located south of I-94 in Milwaukee County will be visited for viewing Tuesday and Thursday, if the request is received before 8 a.m. on that day. Requests received after 8 a.m. will be viewed on the next scheduled viewing day.

*If your funeral home is located outside of Milwaukee County, the body has to be brought to the Medical Examiner’s Office for the viewing.

If a funeral home plans on bringing a body to the Medical Examiner’s Office for a cremation view, they are required to call (414) 223-1200 in advance to notify the investigative staff before their arrival. This applies to all funeral homes. Funeral homes are also required to provide basic information about the deceased at that time of the notification call, OR the Cremation Request form can still be faxed, but please verify by calling the office that the investigative staff received the request prior to arrival.

Permits are issued before 10 a.m. daily if all the required paperwork has been submitted before that time.

All bodies MUST be labeled with some form of identification prior to viewing, or viewing will be denied and the permit will be delayed.

Print and fill out the form below to request a disinterment permit. The disinterment request form needs to be completely filled out and notarized. The cost for a disinterment permit is $50.

Funeral Homes with Outstanding Balances

Pursuant to Wis. Stat. §§ 59.36 and 59.38(1), which authorize the County Board to set fees for all services rendered by the medical examiner and require that such fees be collected by the Medical Examiner, this policy establishes procedures to ensure compliance. The fees may not exceed the amount reasonably related to the actual and necessary cost of providing the service, and Wis. Stat. § 59.365 places additional restrictions on the manner and extent of fee increases.

The purpose of this policy is to establish procedures for funeral homes with significant outstanding balances owed to the Milwaukee County Medical Examiner’s Office (MCMEO). This policy is designed to ensure timely payment for services rendered and to promote equitable and consistent treatment of all funeral homes.
