The Milwaukee County Department on Aging is also the local Area Agency on Aging.

Federal funds provide assistance with home and community based services and programs that affirm the dignity of older adults by supporting their choices for living in and giving to our community. Through funds provided by the federal Older American's Act, the state of Wisconsin and Milwaukee County, we sponsor contracts with local service providers to operate a variety of programs including:

REQUEST FOR PROPOSAL MATERIALS FOR 2018 PROGRAM AND SERVICE CONTRACTS

INTRODUCTION

The Milwaukee County Department on Aging is requesting proposals from qualified community agencies to provide programs and services for the period January 1, through December 31, 2018. Contracts to provide programs or services will be awarded on the basis of a competitive process, and prospective applicants are advised that the submission of a proposal does not obligate Milwaukee County to fund the proposed program or service. Applicants are further advised that the award of contracts is contingent upon the availability of federal, state, and county funds. Each program and service in the following section contains these materials:

  1. Cover letter
  2. Proposal Guidelines and Requirements for Department on Aging Program and Service Contracts: 2018
  3. Section VI, 2018 Department on Aging Program/Service Guidelines or Specifications (one for each program or service area for which proposals are being requested)
  4. Forms Packet to be used for completion of Section VII, Exhibit I (use a Forms Packet for each program or service area in which you intend to submit a proposal)

To be considered for funding, proposals must be submitted in writing on the forms provided by the Department and delivered in person no later than 5:00 p. m., Tuesday, September 5, 2017, at the following address:

Applicants must submit one original proposal and three copies of the proposal for each program or service area for which they are applying.

When older adults and their families need to make decisions about in-home care our long-term care staff provide counseling services and are able to conduct in-home assessments to determine need, provide information and services to meet those needs, develop appropriate care plans, and arrange for funding for services (when applicable).
