Health Assessments - Current Employees

Health Assessment Process for Current Employees  (hired before 2/9/2018)

As part of the wellness program, all employees have the opportunity to have a health assessment. A health assessment gives an individual a snapshot of their current health, which helps them set health related goals. Employees who participate in the health assessments will receive a 30% reduction to their monthly premium (up to a maximum $50/month). Spouses and partners are not required to participate in order for the employee to receive the premium reduction.

 

Workforce Health will be conducting health assessments onsite at Milwaukee County facilities from January 8th through March 5th. Communication regarding the annual health assessments were sent out by Workforce Health in mid-December. You can find the information sent below:

 

In order to be considered as having completed a health assessment and earn the incentive, employees must complete:

1. An online questionnaire

2 A biometric health screen

 

Employees who participated in the health assessment will receive a score, based on their health screen results, between 0 and 100. An employee can log back into their Wellness Portal three business days after their health assessment to see their score and determine if health coaching is necessary to maintain the insurance premium reduction.

Online Questionnaire

The questionnaire should only take you about 15 minutes and will ask you questions related to your health behaviors

  • Visit the Wellness Portal at www.workforcehealth.org/mkecounty to register and complete the questionnaire.

  • Once at the portal, locate the "New Users" box and click on register.

  • In the "New User Registration", enter your information as prompted.

    • Your company code is: 0045

    • Your User ID is your 6-digit clock number

    • Enter a valid email address (e-mail is needed to reset your password)

  • Click "Register" to view your home page. Locate the "Questionnaire" box and click "Start" to begin questionnaire.

 


 

 

Biometric Health Screen

The biometric health screen will take approximately 30 minutes. For accurate results, please remember to fast for your biometric screening appointment. Fasting means nothing to eat or drink, except water, 10 to 12 hours prior to your appointment.  The health screen consists of:

  • A finger stick blood test for cholesterol and glucose

  • Height

  • Weight

  • Waist circumference

  • Blood pressure 

  •  A one-on-one meeting with a Froedtert Workforce Health’s health coach to review the results

 

There are two options for completing the health screen:

  1. Schedule an appointment at a County facility by visiting www.pickatime.com/mkecountyhra. Employees can visit any location to have their screening completed. You can also access this link by logging into the wellness portal and locating the "Health Assessment" box on the home page.

  2. Take this form to your primary care physician and have them complete it. Your physician will need to fax or mail the form to Workforce Health. You will receive an email from Workforce Health confirming they received your form. They will also ask you to schedule a telephonic appointment to review your results.

 


 

 

Health Coaching Information

Health coaching helps individuals prioritize what is important to them regarding their health and encourages goal setting to achieve an individual’s optimal health. Milwaukee County works with Workforce Health to offer health coaching to any employee. All health coaching is done over the phone and a typical appointment is about 15 minutes long.

 

Employees who participated in the health assessment will receive a letter from Workforce Health. This score will be between 0 and 100 and it is used to help individuals better understand their health risks.

  • Employees who score below a 70 on the health screen are required to work with a health coach once a quarter in order to continue earning wellness premium rates

  • Employees who score 70 or above are not required to do coaching in order to receive the wellness premiums, but are welcome to coach regardless

  • Coaching appointments can be made at www.pickatime.com/mkecountycoach or by call Workforce Health at 414-777-3446

  • If an employee is required to do health coaching because of their score from the health assessment they must complete it by the quarterly deadline. The quarterly coaching deadlines are June 15th September 15th, and December 15th.

  • All employees are welcome to work with a health coach regardless of how they scored on their health screen.

Important information

The program is voluntary and participation based. Therefore;

  • If you set a goal during your health screening or in a coaching session and do not meet it you will not be penalized.

  • As long as you are participating and meeting the requirements you will receive the incentive.

 

All health assessments completed by Froedtert Workforce Health are strictly confidential. The County will not receive information regarding an employee's health assessment.
