Health Assessment Process for New Employees

- All employees have the opportunity to participate in a health assessment.

- A Froedtert Workforce Health health assessment:

  • Is made up of two parts: an online questionnaire and biometric health screen
  • Provides each employee with a snapshot of his or her current health
  • Helps when setting health-related goals
  • Makes employees eligible for the wellness premium reduction (30% up to a maxim of $50 off the monthly medical insurance premium)
  • All participants will receive a letter explaining their health risks
  • Are available, but not required, for spouses and partners
  • All participants receive a letter confirming their completion of the assessment. The letter contains a score between 0 and 100 to help the individual better understand their health risks.

 

All new hires will automatically receive the wellness premium reduction for the first quarter their benefits become active. In order to continue receiving the incentive after the quarter is over the employee will need to complete the health assessment.

Online Questionnaire

  • Asks you questions about your health behaviors

  • Should only take about 15 minutes to complete

  • Call Froedtert Workforce Health at 414-777-3446 to obtain your personalized password

  • Visit www.workforcehealth.org/milwaukeecounty and use your ID (0045 followed by your clock number) and personal password to log in

  • The questionnaire is confidential and the County will not know your results

 


 

 

Biometric Health Screen

The biometric health screen will take approximately 30 minutes. For accurate results, please remember to fast for your biometric screening appointment. Fasting means nothing to eat or drink, except water, 10 to 12 hours prior to your appointment.  The health screen consists of:

  • A finger stick blood test for cholesterol and glucose

  • Height

  • Weight

  • Waist circumference

  • Blood pressure 

  •  A one-on-one meeting with a Froedtert Workforce Health’s health coach to review the results

 

There are two options for completing the health screen:

  1. Schedule an appointment at a Workforce Health's facility by calling 414-777-3446. Workforce Health is located at W129 N7055 Northfield Drive Building B Menomonee Falls, WI 53051. Upon approval of the employee's supervisor, they are able to use County time to complete the health assessment.

  2. Take this form to your primary care physician and have them complete it in its entirety. Your physician will need to fax or mail the form to Workforce Health. You will receive an email from Workforce Health confirming they received your form. They will also ask you to schedule a telephonic appointment to review your results.

 


 

 

Health Coaching Information

Milwaukee County works with Workforce Health to offer this free service to employees. All health coaching is done over the phone and a typical appointment is about 15 minutes long.

 

An employee may be required to do coaching in order to continue receiving the reduction in their premiums. The requirement to do coaching will be based on the score they receive from the health screen.

  • Employees who score below a 60 on the health screen are required to work with a health coach once a quarter in order to continue earning wellness premium rates

  • Employees who score 60 or above are not required to do coaching in order to receive the wellness premiums, but are welcome to coach regardless

  • If an employee is required to do health coaching because of their score from the health assessment they must complete it by the quarterly deadline. The quarterly coaching deadlines are June 15th September 15th, and December 15th.

Coaching appointments can be made at www.pickatime.com/mkecountycoach or by call Workforce Health at 414-777-3446. Health Coaching is a free and confidential service that is part of the employee wellness program. All employees, regardless if their on the health plan or their health assessment score, are welcome to work with a health coach.

Special Note!

  • Remember, all aspects of the County Wellness program are voluntary.
  • If you set a goal during your health screening or in a coaching session and do not meet it you will not be penalized.

  • As long as you are participating and meeting the requirements you will receive the reduction in your health insurance premium.

 

All health assessments completed by Froedtert Workforce Health are strictly confidential. The County will not receive information regarding an employee's health assessment.
