How to Apply for a Passport

The Milwaukee County Clerk's Office is an acceptance agent for U.S. Passport applications. Passport photos are also available at a cost of $10.00. Passport applications are taken during the hours of 8:00 a.m. to 4:00 p.m., Monday through Friday.  All persons applying for a passport, including children must be present during the application process.

 

The cost of a passport for adults (persons 16 years of age and older) is $100.00. You will need two checks -- one for $75.00 made out to the United States Department of State and the second made out to the Milwaukee County Clerk for $25.00. The cost of a passport for a child under the age of 16 is $85.00 (one check for the U.S. Dept. of State for $60.00 and one to the County Clerk for $25.00). Both parents must sign the application of a passport for a child under the age of 16.

 

Expedited passport service is available for an additional fee of $60.00 for each passport, plus the cost of overnight mail ($17.50).  If you choose the expedited service, you can expect your passport in about two to three weeks.

 

All applicants must bring in a certified birth certificate and a valid government issued photo identification card (i.e., driver's license). Applicant must provide a photocopy of photo identification card (along with original ID) which must be presented at time of application. (photocopy must be front and back of ID on same side of 81/2 x 11" white paper)  Married women, who have changed their last name, will also need to bring in a marriage certificate.

 

Passport cards are also available. This document can be used to enter the United States from Canada, Mexico, the Caribbean, and Bermuda at U.S. land border crossings or sea ports-of-entry.  To apply for a passport card, you will need the same documents as with a passport.  The fees for passport cards are $45.00 ($20.00 to the U.S. Department of State and $25.00 to the County Clerk) for Adults and $35.00 ($10.00 to the U.S. Department of State and $25.00 to the County Clerk) for children under age 16.
