 Written Reprimands

The purpose of a written reprimand form is to provide an employee with notice that improvement is needed in a particular area.  This is a general form and is not intended to replace any currently used department reprimand forms.

 Suspensions

ALL suspension forms are to be filed with the Milwaukee County Personnel Review Board within three (3) days of the issue date.  Employees are entitled to a hearing before the Personnel Review Board if:

  • This is the second time the employee is being suspended within a six (6) month time frame (time is counted from issue date not date suspension served)
  • The employee is being suspended for eleven (11) days or more

Employees who wish to waive their right to a hearing before the Personnel Review Board and accept the suspension discipline as outlined by the appointing authority may do so by completing the Waiver of Suspension Form.

 Grievances

There are three (3) separate forms used in processing a grievance: the grievance initiation form, the grievance disposition form, and the grievance appeal form.

 

All forms are to be prepared in quadruplicate except at the county institutions, park commission, and department of public works, where five (5) copies are to be prepared. Two (2) copies are to be retained by the person originating the form: one (1) copy will be sent to the other person involved in the procedure at that step, for example, immediate supervisor, and one (1) copy will be forwarded to the Personnel Review Board. At the county institutions, one (1) copy will be forwarded to the office of the director; in the park commission, one (1) copy will be forwarded to the office of the general manager; and public works, one (1) copy will be forwarded to the office of the director.

 

​The grievance process has multiple steps, and a verbal grievance must be made prior to filing a grievance initiation form.​ Please ensure to follow the steps as outlined in Chapter 17.207 of the Milwaukee County General Ordinances. We have also illustrated the steps in the attached flow chart, but be aware the language of Chapter 17.207 controls.

 

The Personnel Review Board presides over the final step of the Non-Represented employee grievance.

 Voluntary Resignations This form MUST be signed and dated by the employee PRIOR to submission to the Milwaukee County Personnel Review Board.  Submit this form BEFORE the scheduled hearing date for proper processing.
