The purpose of a written reprimand form is to provide an employee with notice that improvement is needed in a particular area. This is a general form and is not intended to replace any currently used department reprimand forms.
ALL suspension forms are to be filed with the Milwaukee County Personnel Review Board within three (3) days of the issue date. Employees are entitled to a hearing before the Personnel Review Board if:
Employees who wish to waive their right to a hearing before the Personnel Review Board and accept the suspension discipline as outlined by the appointing authority may do so by completing the Waiver of Suspension Form.
