Milwaukee County Sheriff's Office Online Crash Reporting Form
The purpose of this form is to report an accident that occurred on the Milwaukee County Freeway System within 24 hours of its occurrence during a weather emergency. The MCSO will only complete an accident report if you alert us of the accident within 24 hours and if it occurred during a weather emergency. If your accident happened over 24 hours ago, you may print off a State Patrol self-report form at: http://www.dot.wisconsin.gov/drivers/forms/mv4002.pdf and mail it to the Wisconsin State Patrol.
