Family Care Partnership Provided by Community Care Inc.

Family Care Partnership Provided by iCare (available Jan 2010)

PACE (Program of All inclusive Care for the Elderly) Provided by Community Care Inc.

IRIS (Include, Respect, I Self Direct) A Wisconsin Waiver Program that allows you to direct your own long term care services. 


 To find out if you are eligible for benefits, you must contact the Aging Resource Center and arrange for a visit from a social worker, who will conduct a review to determine your care level needs and financial eligibility.
