INFORMATION FOR FAMILIES
Welcome to Wraparound Milwaukee
A Specialized Managed Care Program run by the Child's Branch of the Milwaukee County Mental Health Division, Wraparound Milwaukee provides care and support to families with a child or children who have serious emotional or mental health needs. Wraparound Milwaukee utilizes a family-centered "Team" approach in working with families. In addition to case management services, Wraparound Milwaukee provides a broad range of youth and family support services including traditional behavioral health services, alcohol/other substance abuse counseling and non-traditional support services such as Parents Assistance and Daily Living Skills.
Wraparound Milwaukee administrative offices are located at 9201 Watertown Plank Road in Milwaukee.
GETTING STARTED
Enrollment in Wraparound Milwaukee is voluntary. Some Youth enrolled in Wraparound Milwaukee have a Court Order to participate in the program. Other's enroll based solely on family choice (REACH Program).
CARE COORDINATION
Local health care agencies provide Care Coordination (case management) services for Wraparound Milwaukee families. Wraparound Milwaukee tries to match the youth and family to a Care Coordinator that will work best based on their needs. You can expect to hear from you Care Coordinator shortly after enrollment in the program.
One of the first tasks for the Care Coordinator is to set a their first meeting with you to discuss their role and talk about the what you can expect from the Wraparound Program.
CHILD AND FAMILY TEAM MEETINGS
Within your first month in the program, your Care Coordinator will set a "child and family team meeting" to begin the process of developing a "plan of care" which includes identifying family strengths and needs. Plan also includes putting together detailed plan to help manage crisis situations. Part of the planning process includes identification and authorization of services that will help the enrolled youth and family meet the needs that have been identified in the plan of care.

CRISIS PLAN
JOIN US FOR FAMILY ORIENTATION
Along with representatives from Families United of Milwaukee (family advocates), staff from Wraparound Administration offer an Orientation for newly enrolled families. Orientation meetings are held every 6 weeks. At the meeting, you will receive detailed information about the Wraparound Program and what you can expect while your child is enrolled in the program.
