Becoming a Service Provider

 

Milwaukee County Department on Aging’s Family Care – Care Management Organization (MCDA – CMO) contracts with the Wisconsin Department of Health and Family Services to provide services for Family Care members.

The Family Care Provider Network

MCDA – CMO contracted provider’s sign a standard contract plus any applicable contract addenda specific to the service criteria for care and quality established by the MCDA – CMO.

Providers must be contracted with MCDA – CMO and obtain prior authorization from MCDA – CMO for goods and services provided to Family Care members.

Contracted providers are listed in our Provider Network directory. MCDA – CMO Family Care members and their care management team choose service providers from this directory.

The MCDA – CMO is required to monitor the Provider Network to ensure we have enough service capacity to meet the needs of current and anticipated members. If there are too many service providers in the Network, our administrative costs increase and our ability to monitor quality decreases. The MCDA – CMO may close the Provider Network in certain service areas when the capacity and demand for services are equal.

Based on our current needs in the network, we are currently accepting applications from specific provider types. We are looking for the following type of providers:

 1.                  State of Wisconsin DHFS licensed Community Based Residential Facility (Class C Non Ambulatory) with a capacity of 9 beds or more.

2.                  Skilled Nursing Facility.

3.                  WE ARE CURRENTLY CONSIDERING REQUESTS FOR APPLICATION FROM AGENCIES PROVIDING SERVICES FOR THE MILWAUKEE COUNTY DISABILITY SERVICES DIVISION, CLICK HERE TO REQUEST APPLICATION.  

If you are a provider who meets the criteria in one of these categories listed above and are interested in working with the MCDA-CMO, please send a copy of your license with a letter explaining your experience and describing your facility to:
