Most child support services are free to applicants.* To apply for child support services, print the Application for Child Support Services.** After you have entered all of the requested information, mail or bring the completed form to:

If you are unable to print the Application for Child Support Services you may call Customer Service at (414) 278-5160 and request that a form be mailed you.
*General child support services are free, but participants may be assessed fees for specialized services provided.
