Milwaukee County Department on Aging Contracts for 2012

THE RFP APPLICATION DEADLINE FOR 2012 PROGRAMS AND SERVICES IS NOW CLOSED

The deadline for submission was 5PM September 6, 2011.

 


Milwaukee County Department on Aging (MCDA) posts requests for proposals (RFPs) each year for a variety of programs and services assisting elders 60 or older.  When the application process is underway, RFPs are available at the Department on Aging in hard copy and also on Milwaukee County's Business Opportunities web portal. Information and links to required paperwork are posted on this website when RFP's are being accepted.

 A similar list of programs and services to be implemented in 2013 will be posted in August 2012.


Depending on available funding and other factors, the proposal format and other requirements may change for 2013 or in future years.  Whenever considering submission of a proposal make sure you have reviewed all RFP requirements and/or contacted the Department on Aging for additional information.


The programs out for RFP for 2012 included the following:

 
Services to Asian American Elderly
Program activities, which the applicants must provide include, but are not limited to site management, food service, social service activities, special events, record keeping and program data information collection and compilation, advocacy, and site monitoring. Applicant must apply for the full amount of funds allocated for this program. Applicant must be able to administer the entire program without subcontracting. Access and Outreach Services SE Asian American Elderly Socialization Education and Recreation
Develop, promote, organize, maintain and provide educational, cultural, social, and recreational activities designed to lessen the isolation of Southeast Asian American elders, with emphasis on serving low-income older adults. Serve as a meal site for the Milwaukee County Senior Meal Program. Conduct outreach activities to identify culturally or physically isolated Southeast Asian American elders. Refer such persons to appropriate services provided through the Department on Aging and other public and private agencies, and assist such persons in taking advantage of benefits under programs for which they are eligible. Cooperate with, and assist the Department on Aging Information and Assistance Unit in its efforts to serve elderly members of Milwaukee County's Southeast Asian communities. Applicant must apply for the full amount of funds allocated for this program. Applicant must be able to administer the entire program without subcontracting. Access and Outreach Services SE Asian Elderly Translation and Interpretation
The provider of this service must employ at least one full-time direct service worker to provide the following activities and services: Acting as an interpreter and advocate for non-English speaking Southeast Asian American elders for routine health care, special services such as simple legal matters and in accessing programs and services available to older adults. Conducting outreach activities to identify culturally or physically isolated Southeast Asian American older adults, referring such persons to appropriate services provided through the Department on Aging and other public and private agencies. Assisting such persons in taking advantage of benefits under programs for which they are eligible. Cooperating with and assisting the Department on Aging's Information and Assistance Unit in its efforts to serve elderly members of Milwaukee County's Southeast Asian communities. Assisting the Department on Aging and other agencies in gathering information regarding the overall needs of Milwaukee County's Southeast Asian American elders or specific communities of Southeast Asian American elders, Cooperating with the Department on Aging and other agencies serving the elderly in receiving ongoing training in the programs and services available to older adults, including the eligibility and access requirements for these programs and services. After Hours Telephone Coverage to Aging Resource Center
The applicant must have experience providing call center operations to persons 60 and older and must be able to recruit qualified staff by January 2012 and to fill vacant positions within one month of their occurrence. The applicant must have prior experience working collaboratively with large government agencies and experience complying with complex federal and state funding program compliance guidelines. Applicant must apply for the full amount of funds allocated for this program. Applicant must be able to administer the entire program without subcontracting. Benefit Specialist Legal Services
Provide Benefit Specialist/Legal Services to older adults using at least five paralegals, with supervision and consultation from an attorney as required. Applicants must demonstrate the ability to provide legal services through experience and capacity to deliver legal assistance, including experienced staff and the capacity to provide: Effective administrative and judicial representation, Support to other advocacy efforts and legal services to institutionalized, isolated, and homebound older persons. Case Management and Delivery Services Home Delivered Meals
Provider responsibilities include, but are not limited to the following: Client assessments/reassessments, meal delivery, routing, driver supervision, case management and service coordination, ordering of meals on a daily basis from caterer(s) designated by MCDA, monitoring of meals to ensure compliance with specifications and regulations, coordination of meal distribution at seven dispatch locations Monday through Friday, except most Major Holidays. Coordination of Neighborhood Services
Plan, coordinate and enhance the voluntary outreach and supportive services provided to older adults by neighborhood and community organizations, including religious congregations. Applicants must describe how they will move toward creating neighborhoods of connection as demonstrated through Connecting Caring Communities. Such services are to include: identification of homebound, vulnerable older adults in need of supportive services; determination of specific services needed; provision of needed services or referral to appropriate agencies to provide these; and follow-up to see if needed services have been provided. Family Caregiver Support and Alzheimer’s Direct Services
Work with public and private sectors to access family caregiver support resources and provide direct services to those under the Alzheimer’s Family Caregiver Support Program. Applicant must apply for the full amount of funds allocated for this program. Applicant must be able to administer the entire program without subcontracting. Late Life Counseling Services
Provide individual counseling by a social worker to Milwaukee County older adults in the clients home. The primary goal of LLC is the amelioration of stress in the older adult with the improvement of coping mechanisms. Special areas of concern will include: isolation and loneliness, depression, physical and mental decline, separation anxiety and loss, fear of death, changing role and self-concept, changes in interpersonal relationships, prejudice against the ages, and financial problems. Nutrition Site Supervision at Lakeside Senior Enrichment Program
Program activities which the applicants must provide include, but are not limited to nutrition site management at Lakeside Senior Enrichment Center, food service, social service activities, special events, record keeping and program data information collection and compilation, advocacy, and site monitoring. Lakeside Senior Enrichment Center will be a sole source provider for their own catered kosher meals and will be held to the standards as described in the attached 2012 specifications for Weekday Catered Meals for the Senior Meal Program. Menus are to be written by Lakeside Senior Enrichment staff, however, require prior approval from the Senior Meal Program Director to monitor compliance with the Older Americans Act Meal Program Requirements. Nutrition Site Supervision at Milwaukee Christian Center
Program activities which the applicants must provide include, but are not limited to nutrition site management at Milwaukee Christian Center, food service, social service activities, special events, record keeping and program data information collection and compilation, advocacy, and site monitoring. This meal site features an Asian Style Menu prepared on site. The 2012 Program guidelines and specifications for Nutrition Sites must be addressed within the proposal submitted in order to demonstrate that requirements will be met to operate a meal site. A separate budget is to be submitted for the catering component of this contract. Applicant must apply for the full amount of funds allocated for this program. Nutrition Site Supervision at Hart Park Senior Center
Applicants must provide program activities at the Hart Park Senior Center including, but not limited to nutrition site management, food service, social service activities, special events, record keeping and program data information collection and compilation, advocacy, and site monitoring. Applicant must apply for the full amount of funds allocated for this program. Applicant must be able to administer the entire program without subcontracting. Outreach and Services to Lesbian Gay Bisexual and Transgender Elderly
Provide outreach and program information to LGBT older adults. Work effectively to meet the unique service needs of LGBT older adults. Provide educational programs to service providers within the Milwaukee community about the unique needs of LGBT older adults. Coordinate this program with other agencies providing services to LGBT persons, particularly for client referrals. Coordinate this program with the Department on Aging case managers and community partners when needed. Demonstrate strategies for resource development and fundraising to enhance programs. Assist clients in taking advantage of benefits under other programs. Implement educational programs and forums for groups of LGBT older adults, (such as the Sage/Milwaukee general membership) designed to increase awareness and knowledge of available services, benefits, and programs for older adults. Include access to information, links and support for LGBT older adults through an agency web site. Applicant must apply for the full amount of funds allocated for this program. Outreach in Public Housing
The applicant must have experience providing Outreach services to persons 60 and older and must be able to recruit qualified staff by January 2012 and to fill vacant positions within one month of their occurrence. The applicant must have prior experience working collaboratively with large government agencies and experience complying with complex federal and state funding program guidelines. The applicant must be able to provide outreach services in accordance with all state and federal guidelines. Applicant must apply for the full amount of funds allocated for this program. Programs in Five Community Senior Centers
The provider of Programs in Clinton and Bernice Rose, McGovern Park, Washington Park, Wilson Park, and Lawrence P. Kelly Senior Centers must provide a comprehensive program of services and activities for older adults. To be eligible to provide services under this program, an applicant must be legally incorporated under the laws of the State of Wisconsin, and have as its primary purpose or business the provision of services to older persons. Applicant must apply for the full amount of funds allocated for this program. Applicant must be able to administer the entire program without subcontracting. Services to Native American Elderly
Program activities which the applicants must provide include, but are not limited to site management, food service, social service activities, special events, record keeping and program data information collection and compilation, advocacy, and site monitoring. Applicant must apply for the full amount of funds allocated for this program. Applicant must be able to administer the entire program without subcontracting. Shopping and Errand Services
The provider of this service must perform shopping and errand services for homebound clients. The service area is limited to the geographic boundaries of Milwaukee County, unless approved in advance by the Department on Aging contract manager. Telephone Reassurance Services
Provide Telephone Reassurance Services to Milwaukee County’s older adults who are unable to receive Telephone Reassurance Services through any agency and who are currently not receiving other daily services such as Home Delivered Meals, etc..
